Rochdale Borough Council

Connecting You aims to build healthier lives in the Rochdale Borough Council area by bringing a wide number of teams and services to work together to help residents to access the information, advice and support they need to make better choices and achieve their wellbeing aspirations. Connecting You recognises that people’s health is determined by a range of social, economic and environmental factors and makes time for conversations with residents, whether about handling debt or housing problems, lifestyle change or volunteering. The model encompasses four elements: integrating and maximising current and new services to improve access to support; building and utilising community assets, strengthening the voluntary and community sector; investing in a directory of services, tools to aid behaviour change, and workforce development; and proactively reaching out to identify those who might benefit most. Connecting You commenced in the summer 2018 and aims to reach our most ‘at risk’ 20,000 residents over the next three years.
