The Gateshead Care Partnership has created an alliance between Gateshead Council, Gateshead Health Foundation Trust, Northumberland Tyne and Wear Trust and Community Based Care (GP Consortia) to provide co-ordinated and responsive community care to residents in Gateshead, overcoming the previous fragmentation and duplication within service provision. The collaborative working between Local Authority Domiciliary Care teams and Foundation Trust Locality Teams as well as the highly effective operation of a multi-disciplinary social and healthcare team at Eastwood Promoting Independence Centre has led to Gateshead supporting 89.2% of people to still live at home 91 days post hospital discharge period – 3.4% up on the same period in 2017.  The Partnership was recently recognised at the prestigious Health and Social Journal awards winning the ‘Improved Partnerships between Health and Local Government award’ in November 2018.
